Sliding Fee Scale

All Chestnut Family Health Center patients have access to a sliding fee scale. Your family size and income determine whether your care will be billed based on the scale. When you become a patient, our staff will help you to learn whether you qualify. No patient will be denied services based solely on the patient’s inability to pay. You will need to provide the following:

  • Identification: Acceptable forms include a State driver’s license or identification card, passport, or similar identification from other countries of origin.
  • Family size: Includes dependent children, spouse/partner, children up to age 26 who are attending college*, and any individual for whom the applicant has legal responsibility and who is shown as a dependent on their income tax filing.
  • Income: Please present one month’s paycheck stubs for each family member working and living with you.

We will re-evaluate this information every 12 months. If your family size or income changes at any time, please let us know.

Insurance

We accept health insurance from a variety of companies as well as Medicare and Medicaid. We also care for patients who do not have health insurance. If you have private insurance, please check with your insurance provider before your first appointment to see whether Chestnut is in network.

If you have insurance that includes a co-payment, please plan to pay that amount at the time of service.

Please bring your insurance card with you to each visit.
