Chestnut Family Health Center has two convenient centers: Bloomington and Granite City. Both provide primary care, laboratory services, pharmacy, screening and treatment for mental health issues, routine physicals, and OB/GYN services, regardless of your ability to pay.

Sliding Fee Discounts
Chestnut Family Health Center is a community-based health center. Access to a sliding fee scale for patients who qualify based on family size and income is available. Chestnut's financial intake staff are happy to meet with you to review eligibility requirements and establish a sliding fee rate.
We offer services at discounted rates for patients who qualify based on their family size and income. Co-payments are collected at the time of services. Insurance co-pays are shown on your insurance card. We accept the following insurance:
- Health Alliance
Insurance Coordinators are available to assist with insurance eligibility questions and enrollment through the Illinois and Federal Insurance Marketplace. All services are confidential. Patients are seen regardless of the insurance coverage or ability to pay.
Chestnut Family Health Center provides interpreting services, available by phone so that we can better service our patients who do not speak English. Sign language interpretation is also available. If you need these services, please let us know 48 hours before your appointment.
