Our Financial Intake Specialists will help you through the steps of determining fees including checking your insurance benefits. Payment at the time of service is preferred.

Chestnut accepts medical insurance, Illinois Medicaid, and direct client payment by cash, check or credit card. All patients/clients have access to a sliding fee scale. Your family size and income determine whether your care will be billed based on the scale. Chestnut can also help with loans. For some clients, state-funded financial assistance may also be an option. We can help you determine your eligibility.

We accept health insurance from a variety of companies as well as Medicare and Medicaid. Please check with your insurance company to see whether Chestnut is in network. We also treat patients without insurance. Call us for details.

Although you may complete forms prior to your visit, all signatures must be made and witnessed at the time of your intake appointment.
